Descripción del trabajoAll positions are located in Fresno / Clovis CA Serving as a liaison between Community Medical Centers and the community by initiating, intervening, and evaluating physical therapy services to patients in their home. Implement the care plan, including patient and family education, in collaboration with other team members. Evaluating, planning, organizing, and implementing specific treatment programs for individual patients. Implements therapy according to established plan, utilizing appropriate procedures, equipment and techniques within the scope of physical therapy. Educates the patient and / or the caregiver about the findings of the evaluation, established goals of the physical therapy program, plan of treatment, home programs, injury prevention, and other related information. Communicates effectively with other patient care team members and coordinates care provided. Supervises PT Assistants with implementation of treatment plan. Bachelor's Degree in Physical Therapy Valid California Driver's License and driving record acceptable to Community Medical Centers required Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) 1 year of experience in an inpatient or outpatient clinic setting Pay ranges listed are an estimate and subject to change. If any bonuses are noted, they are only applicable to external hires meeting criteria.
