Responsibilities Work in a home health and hospice care setting Variable shifts with 8-hour day and evening hours; occasional night shifts may be required May be on-call for night shift phone calls or home visits Expected to float to different units as needed Responsibilities include initial patient assessments, care planning, providing skilled nursing care, and discharge planning Involvement in developing and managing Home Health Aide care plans Coordination and collaboration with the interdisciplinary team to ensure continuity of care Responsible for accurate and timely clinical documentation in compliance with regulations Support and assist with implementation of family bereavement services Must be fully vaccinated against COVID-19 (booster not required) Not accepting local candidates within a 60-mile radius Return staff must have a minimum one-year separation from previous employment at the health system Up to 3 days of requested time off may be considered for auto offers; up to 7 days for standard offers, to be approved prior to signing Occasional holiday and rare night coverage may be required Requirements Active WA or compact RN license, or proof of application for WA state license Minimum 1 year of home health or hospice / end of life nursing experience required Current BLS certification required Valid driver's license and proof of auto insurance required Minimum 1 year of recent Epic EMR experience required Graduation from an accredited school of nursing required; BSN preferred Demonstrated ability to apply age-specific principles of growth and development in patient care Basic computer skills, including familiarity with MS Office applications Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-619529.
