Location : 365 West Passaic Street, Rochelle Park

FT 40 Hours : 8 : 00 am - 4 : 00 pm / 9 : 00 am - 5 : 00 pm; 1 late shift per week on an as needed basis (10 : 00 am - 6 : 00 pm)

Salary Range : $46,000 - $48,000

Job Summary :

You will provide care coordination between physical and behavioral health providers and social support services. Additionally, as the case manager, you will enhance navigation between providers and empower individuals to achieve their best health by ensuring their healthcare needs are met.
