HSS is a private, non-profit community resource agency founded in 1973, providing confidential COA-accredited services.

Human Support Services is seeking to add a full-time Case Manager, to our premier team of mental health professionals to provide mental health treatment in Monroe County, IL. This position works with our supported residential housing clients / population.

ILLUSTRATIVE EXAMPLES OF POSITION DUTIES :

Case Management
Advocacy, referral and linkage to Community Support Services
Community Education
Using community and resource knowledge to engage clients
Documentation of clinical services
Meet agency standards for billable services
Utilize a variety of therapeutic interventions to address clinical needs.
Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, assessments, treatment / recovery plans, progress notes, treatment authorizations and authorization of medical necessity.

Requirements :

Minimum of a Bachelor's degree in a Human Services field (Psychology, Social Work, Counseling, etc.)

One year relevant job experience in case management preferred.

Must be willing to work in a multidisciplinary team environment.

Must be a self starter and able to manage time

Must have a valid driver’s license and be insurable for auto coverage.
