• Home Health OT Position Summary

As a

• Home Health OT
• , you will :
• Conduct
• in-home patient assessments
• to evaluate physical, cognitive, and functional abilities
• Develop
• individualized treatment plans
• based on patient goals and needs
• Deliver
• therapeutic interventions
• to improve
• ADLs (Activities of Daily Living)
• Educate patients and caregivers on
• home safety, adaptive equipment, and self-care strategies
• Collaborate with
• nurses, PTs, and interdisciplinary care teams
• to coordinate care
• Document visits and progress accurately in the EMR system
• Ensure compliance with all
• regulatory, agency, and safety standards

• Qualifications
• Licensed
• Occupational Therapist (OT) in Ohio
• Graduate of an
• accredited OT program
• Minimum
• 1 year of home health experience
• preferred, but not required
• Valid drivers license
• and reliable transportation for home visits
• Strong
• communication, organizational, and time management skills
• Comfortable working
• independently in patient homes
• Passion for
