Home Care RN – Baylor Schedule

As a Home Care Baylor RN at VNA Health Group, you’ll provide expert nursing care to patients in their homes while enjoying a schedule designed for flexibility and work-life balance. This Baylor position works every Saturday and Sunday, 12-hour shifts, and is paid for 30 hours .

What You’ll Do

Deliver Exceptional Care : Provide skilled nursing services and coordinate care for patients in their homes.

Comprehensive Assessments : Evaluate patient needs, create individualized care plans, and monitor outcomes.

Educate and Empower : Support patients and families in understanding treatment plans and self-care strategies.

Collaborate for Success : Work closely with physicians, case managers, and interdisciplinary team members.

Document with Purpose : Maintain accurate, timely documentation through our EMR system.

Qualifications

Current NJ RN license required.

Minimum 1 year of clinical nursing experience; home care experience preferred.

Strong communication, clinical judgment, and critical thinking skills.

Proficient with EMR and digital tools.

Reliable car, valid driver’s license, and proof of auto insurance required.

Working Conditions / Physical Demands

Work primarily in patient homes with occasional office visits.

Use of clinical equipment, computer, and mobile technology.

Direct patient and family contact—your care makes the difference.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000 When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education / training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
