Descripción del trabajoConduct comprehensive psychosocial assessments to evaluate patients emotional, mental, and social needs as part of the overall care plan. Coordinate discharge planning by collaborating with the interdisciplinary healthcare team and connecting patients with appropriate post-hospital resources and services. Provide crisis intervention and short-term counseling to patients and families coping with illness, trauma, loss, or other life-altering events. Advocate for patients and families , ensuring their needs, rights, and preferences are represented and addressed within the healthcare system. Maintain accurate documentation of assessments, interventions, and care plans in compliance with hospital policies and regulatory standards (e.g., HIPAA). LMSW license active and in good standing (REQUIRED) At least 1 year of hospital social work experience (REQUIRED) Be part of one of NYCs top-ranked hospital systems with a reputation for excellence in patient care Make a direct impact on patient outcomes and family support Work with a collaborative, mission-driven team of healthcare professionals Competitive hourly pay with weekly direct deposit
