Descripción del trabajoMonday–Thursday : 8 : 00 a.m. – 5 : 00 p.m. Friday : 8 : 00 a.m. – 2 : 00 p.m. Saturday–Sunday : Closed Chronic disease management (Diabetes, Cardiovascular Disease, Asthma, COPD) Preventive health services (Immunizations, Cancer Screenings, Wellness Checks) Integrated Behavioral Health support Same-day appointments for acute illnesses Pediatric primary care Competitive Salary : $100,000 – $145,000 (commensurate with experience) Paid Time Off (PTO) Comprehensive Medical, Prescription, Dental, and Vision coverage COBRA Rates available Employee Assistance Program (EAP) Basic, Supplemental, and Dependent Life Insurance Accidental Death & Dismemberment coverage Short-Term & Long-Term Disability Plans Retirement Savings Program, including 403(b) options Voluntary benefits : Accident, Critical Illness, and Hospital Indemnity Insurance Universal Life Insurance Garnishments and Child Support assistance programs

About the Position
A Federally Qualified Health Center in Hammond, Louisiana is seeking an experienced Family Nurse Practitioner to join its team. This health center has a long-standing commitment to serving diverse and underserved populations across the region, offering high-quality, affordable care through a comprehensive, patient-centered model.

Requirements :
- 3–5 years of experience as a Nurse Practitioner
- Active Louisiana NP license
- Experience with both adult and pediatric patients
