Description : Klarus Home Health / Jet Health, Inc is seeking a full time, part time or PRN (per diem) dedicated and professional Weekend Admissions Physical Therapists to join our team serving our patients enrolling in our home health services in Denver Metro area of Colorado and surrounding areas.

This role involves providing coordinated care to patients / clients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being. The Physical Therapist will plan, organize, and conduct physical therapy treatment programs based on medical referrals and their evaluations, ensuring effective coordination of care.

Requirements :

Key Responsibilities :

• Develop and implement individualized physical therapy treatment plans.
• Evaluate and document patient / client progress, adjusting treatments as necessary.
• Coordinate care with other healthcare professionals to ensure comprehensive treatment.
• Educate patients / clients and their families on treatment plans, home exercises, and preventative care.
• Maintain accurate and timely patient records in compliance with company and regulatory standards.
• Utilize durable medical equipment as needed and provide training to patients / clients.
• Ensure patient safety and comfort during therapy sessions.
• Participate in continuous professional development and stay updated with the latest therapy practices.
