Family Medicine or Internal Medicine or Geriatrician - Post-Acute Care Wilmington, DE

Position Highlights :

  • Full-time position (5 days / week) providing primary care in 2 skilled nursing facilities and 1 assisted living facility
  • Flexible rounding schedule to create work / life balance
  • Autonomy to manage your own daily schedule
  • Leadership and mentoring opportunities for nurse practitioners
  • Collaborate with interdisciplinary teams, facility staff, and leadership
  • Participate in care coordination initiatives and quality improvement

Job Duties :

  • Provide primary care rounding in SNF and ALF settings
  • Diagnose and manage acute and chronic medical conditions
  • Develop and implement individualized care plans
  • Conduct clinical case reviews and share best practices
  • Foster strong relationships with patients, families, and facility staff

    • Compensation & Benefits :

  • Estimated annual compensation : $210,579 $271,579 (no cap on productivity income)

    • Candidate Requirements :

  • Board-certified or board-eligible physician
  • Experience in post-acute care preferred but not required
  • Active and unrestricted Delaware medical license (or in process)
  • DEA license with prescriptive authority
