Position Details
The RN Care Manager works autonomously, using discretion and independent judgement to provide clients, families, or vulnerable populations with medical and psychosocial support needed to cope with chronic, acute, or terminal illnesses. Includes administering client assessments, care planning, advising families care givers, providing patient education and counseling and making necessary referrals for other social services. Counselor specific duties to include individual or family counseling.
Job Duties
- Complete a thorough assessment of each client and develop a care plan tailored to the client's needs
- Advocate for clients or patients to resolve crises.
- Collaborate with other professionals to evaluate patients' medical or physical conditions and to assess client needs.
- Refer patients, clients, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
- Coach clients and families to help them overcome challenges, recover from illness, and adjust to life.
- Utilize consultation data and social work experience to plan coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
- Coordinate with discharge planners.
- Modify plans of care to comply with changes in clients' status.
- Monitor, evaluate, and record client progress according to measurable goals described in care plan.
- Identify environmental impediments to client or patient progress through interviews and review of patient records.
- Refer Family First services to individuals in need of assistance.
- Accurately document, chart and record clients billing information, billable hours, and client notes.
- Successfully complete all billable expectations and billing requirements.
- Travel as required
Qualifications :
Education (applicants must have at least 1 of the following degrees / certifications) :
Nursing (LPN, RN)Experience :
Two to five years in Nursing, and / or care management experience preferredTwo years' experience in community-based Healthcare (home health, home care, hospice) preferredExperience working with the Geriatric populationCurrent driver's license and auto insurance and the ability to drive to and from clientsCertified Care Manager (required within 1 year of employment)Intermediate or higher level of MS Office experienceAbility to consistently meet billable and documentation requirementsAdaptable with a growth mindsetExperience documenting in an Electronic Health RecordCompensation : Family First is committed to providing market competitive, equitable pay to all employees
Compensation : $50.48 - $55.29 / hr.

Hours : Monday- Friday 8 : 00am - 4 : 30pm - Weekend call rotation every 5-6 weeks
