Your Role

As a key member of our Patient Care Services team, you will work closely with the Nurse Manager to lead and inspire our staff while ensuring the highest standards of patient care. In the absence of the Nurse Manager, you will take charge of patient care activities, unit operations, and staff functions. Your contributions will help us achieve our goal of being a Patient-Centered Medical Home .

Key Responsibilities

• Advanced Assessment Skills : Utilize your expertise to assess healthcare needs through effective interviewing and observation.
• Care Planning : Initiate and implement the Interdisciplinary Plan of Care (IPOC) based on thorough assessments.
• Implementation of Care : Prioritize and deliver care that meets the unique needs of each patient.
• Evaluation of Care : Assess patient responses to interventions and document outcomes accurately.
• Leadership : Serve as a resource for staff, demonstrating advanced knowledge in patient care delivery.

Qualifications

Physical Demands

This role requires prolonged standing, walking, and the ability to lift and move patients of varying weights.

Working Conditions

This position typically follows a Monday through Friday schedule, providing a balanced work-life environment.
