InnovAge PACE (Program of All-inclusive Care for the Elderly) is an alternative to nursing facilities. It provides customized healthcare, social engagement, and exercise to older adults to help them stay independent and in their homes. PACE participants receive most of their healthcare and social support at an InnovAge center in their community. Each person in the program has a team of healthcare experts and a personal care plan that is focused on providing a personalized experience.
As a Psychiatric Nurse Practitioner (PMHNP) you are an integral part of developing and delivering a personal care plan, ensuring high quality, clinically appropriate, team-based behavioral health care. Physical attendance at an InnovAge center(s) is essential to performing the job duties with the option to travel to meet participants outside of center. No on-call or weekend hours required.
You will
- Confidently and swiftly navigate ambiguous situations to manage participants care in real time
- Take bold action using evidenced-based medication management to support acute needs
- Emphasize gradual dose reduction
- Natural ability to integrate with primary care and other Interdisciplinary Team (IDT) members
- Adapt interpersonally to meet participant and IDT clinical needs in real time
- Pioneer InnovAges developing behavioral health integration program
- Serve as mentor / educator to PCPs regarding psychotropic medication management and behavioral change
Clinical Care - 70%
Provide in-person clinical assessments of participants, prescribe and deprescribe, identify treatment protocols, and on-going medication management in centers and via telehealthCoordinate care with Behavioral Health Providers, PCPs, and IDTConsult with IDT staff on behavioral interventions, nursing practice, and psychopharmacology, including writing orders for medications, as appropriateProvide integrated therapeutic careMonitor psychiatric hospitalization care and follow-up disposition planningRespond to urgent calls from clinics, center staff, and facilities, with a solution-focused approachMonitor response to psychotropic medicationsAdministration - 30%
Educates IDT to manage and resolve behavioral complexities or placementsLiaisons with external psychiatric providers to ensure appropriate referrals, continuity of care, monitoring / follow up and report to IDTProvide in-service education at PACE centers; participate in staff meetings as neededContribute to case review, consultation, and utilization reviewResearch and utilize evolving psychiatric treatments, PACE service requirements, and legislationParticipates in IDT meetings, conferences, committees, and events as requestedREQUIRED
Current State issued Psychiatric Nurse Practitioner / PMHNP license2+ yrs in psychiatric care and medication management5+ yrs nursing with at least 2yrs as Nurse PractitionerDEA Certification with full prescriptive authorityProficiency with standard Microsoft Office applications and EMR systemsCurrent CPR certification or the ability to obtain CPR certification in first 30 days of employmentRequires personal transportation, valid state issued drivers license, good driving record and auto insurance as required by law.PREFERRED
