Join us and help bring the joy back to healthcare—one patient, one colleague, and one community at a time. Location and Hours of PositionThis position covers the Beaver Falls, Ellwood City, and Autumn Street offices.  Site hours : Monday 7 : 30 AM - 5 PMTuesday 7 : 30 AM - 5 PMWednesday 7 : 30 AM - 5 PMThursday 7 : 30 AM - 5 PMFriday 8 AM - 2 PM(Traveling to sites is required.) NO WEEKENDS  Compensation Minimum start is $28.00 per hour. Annual Wellness Visits for Medicare patients consist of Initial Preventive Physical Exam visits (IPPE or “Welcome to Medicare”) and Annual Wellness Visits (AWV) both initial and subsequent.  Chronic Care Management (CCM) services consist of monthly activities designed to intensively manage chronic conditions of qualified patients and to coordinate all care that patient receives.  QualificationsRegistered Nurse (RN) : Graduate of an approved school of nursingCurrent unrestricted Pennsylvania Registered Nurse (RN) licenseCurrent CPR Healthcare CertificationAbility to demonstrate competency in nursing skills, assessment and decision makingKnowledge, skill and ability to provide routine immunizations based on CDC and CDPH recommendations.Knowledge, skill and ability to monitor vaccine storage and handling guidelines.Ability to demonstrate self-discipline and work independentlyAbility to work over a wide range of areas in a diverse communityExcellent communication skills both oral and written.Excellent customer service skills.Creative and analytical thinkingAbility to work under pressure calmly and efficiently, coping with variable workloads, intermittent delays and emotionally charged situations What Sets PHN Apart The Primary Health Network is the largest Federally Qualified Healthcare Center (FQHC) in the state of Pennsylvania.
