California Licensed Vocational Nurse

A company is looking for a Patient Enrollment Specialist.

Key Responsibilities

Engage with prospective clients and document interactions in the CRM system, making 60-100 outbound calls daily

Facilitate personalized care planning by conveying client preferences to the care team

Assist clients in completing psychosocial assessments and developing actionable care plans

Required Qualifications

Active California LVN license, Medical Assistant certificate, or Bachelor's degree in a related field with 2+ years of direct healthcare experience

Experience working with vulnerable populations

Comfortable with technology, including video conferencing and electronic health records

Prior experience in a remote role with a secure workspace

Bilingual (English / Spanish) preferred
