In this dual role, you’ll support patients and families through psychosocial care while also leading, training, and advocating for our hospice volunteers. As a Hospice Social Worker – Volunteer Coordinator, You Will :

• Provide psychosocial support to patients and families in accordance with their individualized plan of care
• Conduct social work assessments and provide education, case management, and care planning services
• Offer emotional and resource-based support related to family relationships, cultural / spiritual beliefs, long-term care, and advance care planning
• Monitor and support safe home environments for hospice patients
• Serve as the key liaison and advocate for all volunteer services within your branch
• Assess patient and family needs for volunteer support and assign volunteers accordingly
• Supervise, train, and support volunteers, ensuring files, documentation, and visit logs are current and compliant
• Plan and facilitate ongoing volunteer education, recognition, and engagement

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity.  Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range – $23.75 / hr to $27.95 / hr
