What You’ll Do :
- Round at Skilled Nursing Facilities – Manage chronic and acute conditions for patients in designated Orange County nursing homes.
- Reduce Hospitalizations – Stabilize patients on-site and prevent unnecessary readmissions.
- Lead Goals-of-Care Discussions – Partner with patients and families to ensure treatment aligns with their values.
- Drive Quality & Compliance – Close care gaps, support value-based initiatives, and maintain excellent documentation.
- Collaborate & Lead – Work closely with NPs, nurses, and care teams to deliver seamless, patient-centered care.
Salary & Benefits :
$235,000 - $295,000 base salary (negotiable based on experience) plus a quality bonusBenefits include medical, dental, vision, HSA, matching 401k, generous PTO, and paid holidaysQualifications :
