RN Case Management Consultant in Cookeville, TN
Provide consulting to evaluate and improve case management programs at regional health centers. This role requires three weeks of onsite work in New Mexico and Arizona; the remainder can be completed virtually.
Responsibilities
- Conduct program reviews and site visits to assess care coordination practices.
- Collaborate with clinical staff and stakeholders to gather insights.
- Analyze documentation and prepare clear recommendations and reports.
