    • Job Summary

    We are seeking a part time Adult Case Manager to join our team. In this role, you will work collaboratively with adults with intellectual / developmental disabilities and their families to determine their needs, develop the best course of action, and monitor progress to ensure the delivery of high-quality, individualized support. The ideal candidate is compassionate, patient, and knowledgeable about behavioral healthcare and developmental disabilities.

    Responsibilities

  • Conduct in person, virtual and phone contacts for referrals, meetings and troubleshooting
  • Collaborate with individuals, families and mentors and facilitate communications
  • Coordinate scheduling
  • Develop individualized care plans
  • Work with mentors to monitor progress toward goals
  • 15 to 20 hours per week - primarily mid-afternoon / early evening hours

    • Qualifications

  • Previous experience as a Case Manager or similar position preferred
  • Experience with intellectual / developmental disabilities preferred
  • Strong problem-solving and organizational skills
  • Ability to manage multiple projects or tasks and prioritize appropriately
  • Ability to work in fast-paced situations and make sound decisions quickly
  • Excellent interpersonal skills and high level of compassion
  • Strong verbal and written communication skills
