Plan and implement individualized physical therapy treatment programs using evidence-based practices, collaborating closely with patients, families, and the interdisciplinary care team.
Essential Job Functions:
- Completes patient assessments, develops treatment plans, and documents progress toward goals.
- Implements therapeutic interventions to improve strength, coordination, endurance, self-care, sensory response, mobility, and gait.
- Provides patient and family education and home programs.
- Adapts environment and treatment activities to facilitate patient progress.
- Documents patient care, billing procedures, and productivity per department standards.
- Participates in interdisciplinary team conferences and discharge planning.
- Conducts home evaluations and recommends necessary equipment.
- Provides staff training regarding transfers, mobility, and equipment use.
- Attends required continuing education, works in multiple clinical areas, and contributes to department initiatives.
- Supervises assistants, techs, students, and volunteers as appropriate.
- Performs other duties and special projects as assigned.
