Job Summary :
The Patient Care Coordinator is accountable and responsible for managing the clinical care of course of treatment of a number of patients. Coordination of care is accomplished through collaboration with physicians, associate staff, and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. The Patient Care Coordinator provides supervision and delegation to the nurses, techs, and unit secretaries on their unit The Patient Care Coordinator is accountable to the Patient Care Manager

- Demonstrates self-motivation and the ability to make decisions independently, keeping the PCM informed.
- Demonstrates the ability to assist in the coordination of services with medical staff, nursing staff, and other departments as well as other providers.
- Demonstrates leadership through clinical practice and teaching of nursing staff
- Responds rapidly and effectively during emergency situations.
- Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his / her age specific needs, and to provide the care needed. Serves as a resource person for nurses and other staff on the unit.
- Demonstrates thorough, concise and timely documentation, shift reports, and verbal communication.
- Communicates pertinent patient information to all healthcare team members.
- Maintains own clinical skills staying current in skills and certifications required. Demonstrate the technical aptitude for working and troubleshooting equipment used on medical surgical units
- Communicates effectively with others in an appropriate and timely manner. Adheres to and promotes the established values of the organization, i.e., customer service, safety, and compliance standards.
- Consistently follows patient care and FGH policy, procedures, and standards of care in preforming nursing duties.
- Demonstrates ability to perform all technical skills required for the unit’s population by rendering professional nursing care.
- Demonstrates critical thinking skills and uses professional judgement to respond promptly with intervention when clinical changes occur, and prevention of complications in patients.
- Demonstrates financial responsibility by using supplies in a cost-efficient manner ensuring that patients are discharged in a timely manner, and keeping incremental overtime to a minimum.
- Demonstrates the ability to promote a culture of caring through compassionate care, teamwork, and courtesy in all interactions with patients, visitors, co-workers, employees and non-employees.

Qualifications : Education / Skills
Graduate of a recognized non-online RN program required.
Work Experience :
Preferred work experience of minimum of two years as a staff RN in an acute setting with six months being at Forrest General Hospital.
Certification / Licensure-DUE UPON HIRE
