Descripción del trabajoProvide primary care in post-acute and long-term care settings Manage chronic and acute conditions, prescribe and monitor medications Order and interpret lab and diagnostic results Collaborate with interdisciplinary healthcare teams Educate patients and caregivers on condition management and healthy lifestyle changes Build strong relationships with patients, families, and facility staff Participate in care coordination and promote smooth care transitions MD or DO; Board Certified or Eligible in Internal Medicine, Family Medicine, or related primary care field Active, unrestricted Pennsylvania medical license DEA registration with prescriptive authority in Pennsylvania Minimum of 1 year primary care experience required Post-acute care experience preferred but not required Flexible weekday schedule with no primary night call Uncapped earning potential with a productivity-based compensation model Comprehensive benefits for full-time providers (medical, dental, vision, 401k, life, disability, PTO) Sign-on bonus available at month 1 and month 13 Paid professional liability insurance with tail coverage Opportunities for CME and professional development Leadership pathways and local / national advancement potential Well-supported onboarding, EMR training, and clinical resources Primary Specialty : Internal Medicine or Family Medicine Subspecialty Focus : Post-Acute Care / Geriatric Primary Care
