As a Full-Time Licensed Practical Nurse at VNA of Ohio Hospice Operations, LLC, your daily expectations will revolve around providing high-quality care to our clients in Brooklyn Heights, OH. You will begin each shift by reviewing patient care plans, ensuring you are prepared to meet individual needs. Your responsibilities will include administering medications, monitoring vital signs, and assisting with daily living activities.

Additionally, you will document patient progress and communicate effectively with family members and the healthcare team. Collaborating with Registered Nurses, you will participate in developing and adjusting care plans as necessary. Your role may also involve educating patients and their families on health management and wellness practices.

Maintaining a clean and safe environment for clients will be essential, ensuring that you adhere to all safety and health regulations while fostering a compassionate atmosphere.

• Education: Current and valid Licensed Practical Nurse (LPN) license issued by the state. Completion of an accredited nursing program.

• Experience: At least one year of clinical nursing experience in a hospice or healthcare setting, with knowledge of administering care in patients' homes.

• Clinical Competency: Demonstrated ability to perform skilled nursing tasks, including medication administration, wound care, monitoring vital signs, and assisting with mobility and activities of daily living.

• Regulatory Knowledge: Familiarity with state and federal home health regulations, including HIPAA and CMS's Conditions of Participation, to ensure compliance in patient care.

• Communication Skills: Excellent interpersonal and verbal communication skills to build trust and maintain effective relationships with patients, families, and interdisciplinary care teams.

• Compassion and Empathy: A commitment to providing patient-centered, compassionate care that respects the dignity and independence of each patient.

• Organizational Skills: Strong time management and prioritization skills to handle multiple patient visits and adapt to changing schedules effectively.
