Master Case Manager - Sussex

Location : Sussex County, Laurel, Delaware

Full Time

Pay Rate : $29.76 per hour

Monday through Friday 9am to 5pm, with flexibility for evenings and weekends

Job Summary :

As a Master Case Manager, you will be responsible for providing comprehensive case management services to individuals and families in need. You will assess client needs, develop personalized care plans, coordinate services, and advocate on behalf of your clients to ensure they receive the resources and support they require.

Key Responsibilities :

• Conduct comprehensive assessments to identify client needs and develop tailored case management plans
• Coordinate and monitor the delivery of services, including medical, mental health, housing, and social support
• Advocate for clients to secure necessary benefits, resources, and accommodations
• Maintain detailed case records and documentation to track client progress
• Collaborate with a multidisciplinary team of professionals to provide holistic support
• Continuously evaluate and adjust case management plans to meet evolving client needs
• Provide crisis intervention and emergency assistance as needed
