Role Description

This is a full-time on-site role for a Registered Nurse Case Manager, located in Minneapolis, MN. The Registered Nurse Case Manager will assess patients’ needs, develop and implement individualized care plans, and coordinate care across multiple healthcare services. Additionally, the RN Case Manager will provide direct nursing care, educate patients and families, communicate with healthcare providers, and ensure documentation is accurate. The objective is to promote the best possible outcomes and quality of life for our clients.

Qualifications

• Registered Nurse (RN) licensure in the state of Minnesota
• Experience in case management and patient care planning
• Strong assessment, diagnostic, and clinical skills
• Excellent organizational and time-management skills
• Ability to communicate effectively with patients, families, and healthcare providers
• Proficient in electronic medical records (EMR) systems
• Ability to work independently and make decisions in a fast-paced environment
• Compassionate and patient-centered approach
• Experience with senior healthcare is a plus
• Advanced certifications in case management or geriatric nursing are beneficial
• Bachelor's degree in Nursing (BSN) preferred
