The Position

A Manager of Case Management is needed to oversee day-to-day operations of an acute & ambulatory care management containing about 130 beds. Reporting to the Director of Case Management, this leader will oversee a team of 20+ FTEs. Key responsibilities include maintaining day-to-day operations, continuing to implement process improvements, serving as a liaison with third-party payers, and managing both the Utilization Review and Social Services departments. The ideal candidate will have a strong case management background with leadership experience and the ability to be hands-on with all processes required for utilization and case management. Must be highly organized, patient-focused, and able to adapt quickly to changing needs. Excellent communication skills will be critical. Must be available to start within 2-3 weeks of acceptance.

Requirements

• California RN is required
• Minimum of three years of Care Coordination in acute and ambulatory settings
• Minimum of one year performing
