Description :

This is a full-time position working Monday through Friday for 40 hours per week.

Case managers work either from 7 : 00 AM to 3 : 30 PM or from 8 : 00 AM to 4 : 30 PM based on unit needs.

The responsibilities of case managers include completing assessments on all admitted patients.

They will attend interdisciplinary rounds and initiate referrals for home health, durable medical equipment, skilled nursing facilities, long-term acute care hospitals, and inpatient rehabilitation facilities.

The average daily census is over 1000 hospital beds.

The unit does not accept first-time travelers.

Experience is required in case management within a facility of similar capacity.

Academic experience is preferred.

This role involves high acuity discharges, including discharge planning and coordinating patient care transitions outside of the hospital.

Case managers will discuss the plan of care with patients and their families.

Knowledge of Medicare regulations and appropriate levels of care is essential.

Care progression and expediting care within appropriate time frames for testing are critical components of the role.

Candidates must be willing to float within the service line.

References are preferred at the time of submittal.
