Descripción del trabajoClinical Nurse is a Registered Professional Nurse providing safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care incorporating Relationship Centered Care and nursing process based on theory, research, evidence-based practice, and organizational nursing standards. Assesses and evaluates patient care needs applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in nursing care delivery. Collaborates with interdisciplinary healthcare team in plan of care development, implementation, and evaluation. Prioritizes patient care including teaching, rounding, coaching, planning after hospital care, and delegating as appropriate. Educates patients and caregivers about plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, family, and interdisciplinary team members. Manages assignments within Modified Primary Nursing Care Delivery Model and allocates material resources effectively. Role models Mount Sinai Relationship Centered Care through various patient care actions such as Admission Welcome, Bedside shift Report, HELP AM / PM Care, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD / RN Unit Collaboration. Considers patient's values, preferences, cultural diversity, expressed needs, and knowledge in care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to understand patients' perceptions of care. Initiates service recovery for patient and family concerns and escalates as needed. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff use of evidence-based practices to understand patients' perceptions of care. Conducts Purposeful Hourly Rounding addressing the 4 Ps. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in patient population served. Implements process improvement strategies based on institutional, evidence-based procedures. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams on quality improvement projects, evidence-based practice activities, and nursing research studies. Contributes to quality and safety practices and compliance measures. Implements principles of high reliability to avoid high risk behaviors and provide a safe environment. Escalates potential safety hazards or gaps following institutional chain of command. Utilizes institutional information technology for nursing documentation supporting quality and performance improvement. Performs charge nurse duties as assigned. Coordinates with management to ensure appropriate supply utilization and inventory for patient care. Escalates material or staffing resource deficits to appropriate management. Demonstrates patient-focused and cost-effective approaches to patient care resources. Documents accurately and thoroughly in compliance with hospital, regulatory, and legal requirements and standards of care. Facilitates patient throughput on shift basis. Contributes to environment of care supporting clinical colleagues, patients, families, and healthcare team members. Advances clinical competence in nursing practice from novice to expert. Projects professional image and communicates with caring, respect, compassion, and empathy. Incorporates ethical principles into decision making for patient and family. Encourages and demonstrates scholarship, inquiry, lifelong learning, and innovation. Acquires knowledge and skills relative to role, patient population, clinical specialty, and health community needs. Participates in shared decision making through specialty practice councils, nursing department committees, and unit initiatives. NYS RN License required. BLS certification required. CRRN Med / Surg certification required. Minimum 2 years rehab and med / surg experience required. Minimum 2 years RN experience required. 1 year travel experience required for Travel positions (not required for Per Diem positions). EPIC experience highly preferred. 3x12 hour shifts schedule. Weekends required. Med Surg / Rehab Unit Average Unit Census = 18 24 Beds in Unit Staffing Guidelines = 1 : 6 Med Surg / Rehab Unit - Mount Sinai West Average Unit Census = 18 24 Beds in Unit Staffing Guidelines = 1 : 6
