Wound Care Nurse Practitioner or Physician Assistant

Location : HonorHealth Tempe Medical Center & Osborn Wound Clinic

Full-Time | First Assist Required or Willing to Obtain Within 2 Years

HonorHealth, a nine-hospital non-profit health system serving the greater Phoenix area, is seeking a dedicated and collaborative Nurse Practitioner (NP) or Physician Assistant (PA) to join our Wound Care Service Line . This is a multi-faceted role involving inpatient and outpatient care , as well as First Assist responsibilities in the OR .

This advanced practice provider will work closely with surgeons and interdisciplinary teams to improve outcomes for complex wound patients across two key locations : Tempe Medical Center and the Osborn Wound Clinic .

Primary Responsibilities :

• Provide comprehensive consultation and follow-up for wound surgery inpatients and outpatients .
• Perform First Assist duties for wound care surgical procedures.
• Triage patients requiring higher-level or specialty care to the appropriate services.
• Ensure timely and accurate documentation and charge entry (by end of day; minimum standard within 3 business days).
• Collaborate on safe discharge planning and care coordination to reduce readmissions and length of stay.
• Actively promote the wound care service line across the HonorHealth network and community.
• Provide care to clinic patients with surgical needs , including scheduled visits, walk-ins, and coverage for colleagues as needed.
• Partner with clinic staff to meet performance and patient care goals .
