Descriptif du posteLe Travailleur Social intervient au sein d'une équipe pluridisciplinaire en vue d'améliorer les conditions de séjour des usagers et de favoriser leur insertion sociale. Il participe à l'élaboration et la mise en oeuvre du projet de service, ainsi qu'au fonctionnement général de l'établissement
Assurer l'accompagnement des personnes accueillies et l'accueil en urgence des usagers et renseigner les dossiers des résidents sur l'outil informatique adaptéRéaliser le dossier d'entrée et les démarches administratives afférentesÉlaborer, formaliser, mettre en oeuvre, évaluer et optimiser le projet individualiséPlanifier et mettre en oeuvre des activités pour les personnes accueilliesEffectuer les comptes rendus et assurer la formalisation écrite de l'observationParticiper aux réunions institutionnelles, réunions d'équipe, et réunions de supervision d'équipeParticiper à l'élaboration, la mise en oeuvre et l'évaluation du projet de serviceParticiper à l'évaluation des besoins logistiques et conditions de vie de l'unité d'accueilVeiller au respect et à la bonne utilisation des moyens allouésPréparer et accompagner au départ de l'usagerDévelopper et entretenir le travail de partenariat Tenir à jour les données chiffrées du rapport d'activitéEtre en veille sur l'actualité sociale et juridique
