About Us : Sanctuary Recovery Centers is dedicated to providing comprehensive support and services to individuals and families in need. We are seeking a dedicated and empathetic CORE Case Manager to join our team and make a meaningful impact in the lives of those we serve.

Position Summary : The CORE Case Manager will be responsible for providing case management services to clients, ensuring they receive the necessary support and resources to achieve their goals. This role involves assessing client needs, developing individualized service plans, and coordinating with various service providers. The ideal candidate will have a strong background in social work, excellent communication skills, and a passion for helping others.

Key Responsibilities :

  • Conduct comprehensive assessments of clients' needs and strengths.
  • Develop and implement individualized service plans in collaboration with clients.
  • Coordinate and monitor services provided by external agencies and community resources.
  • Advocate for clients' needs and rights within the community and service systems.
  • Maintain accurate and up-to-date case records and documentation.
  • Provide crisis intervention and support as needed.
  • Participate in team meetings, training sessions, and professional development opportunities.
  • Collaborate with colleagues to ensure a holistic approach to client care.

Qualifications :

  • Minimum of 2 years of experience in case management or a related role.
  • Strong understanding of community resources and social service systems.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Compassionate, empathetic, and committed to client advocacy.
  • Proficiency in Microsoft Office and case management software.
