American Traveler is seeking a travel Home Health Physical Therapist for a travel job in Chippewa Falls, Wisconsin.

Job Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring recent OASIS and HCHB experience and a 1-year minimum background.

Responsibilities

  • Home health position primarily serving geriatric patients
  • Patient caseload averages 5–7 cases per day, subject to confirmation during interview
  • Covers up to a 60-mile radius from the home office
  • Day shift schedule, 5x8-hour shifts from 8 : 00 to 17 : 00
  • On-call rotation every third to fourth weekend is required
  • Uses the HCHB (Homecare Homebase) EMR; proficiency required
  • Travel pairs accepted but each must cover a separate territory with own transportation
  • Flexible coverage of territory is expected
  • Scrubs of any color required; jeans are not permitted
  • iPad provided for documentation, cell phones are not provided
  • Mileage reimbursement available at the standard IRS rate
  • All requested time off must be submitted with initial application
  • Travelers must complete a Hospice / Home Health Checklist
  • Former perm employees must have a minimum 6-month separation and HR approval to return
  • First-time travelers are considered for this position
  • Each traveler must handle their own territory responsibilities independently

    • Requirements

  • Current Physical Therapist license required if applicable
  • Minimum of 1 year of recent experience in home health physical therapy
  • BLS certification required
  • Active driver’s license and personal vehicle required
  • OASIS experience required for those completing Start of Care assessments
  • Proficiency in HCHB system required
  • Timely documentation and communication skills required
