Oregon Licensed Care Management Coordinator

A company is looking for a Care Management Clinical Support Coordinator who will perform duties in support of care management and integrated medical management programs.

Key Responsibilities

Support core functional areas of care management and integrated medical management programs

Engage in phone outreach, care coordination, and clinical support for case and disease management programs

Collaborate within a team environment to enhance patient care

Required Qualifications

Bachelor's Degree in a health-related field or equivalent education / experience

3+ years of experience in clinical, managed care, or care management settings

Motivational Interview training within 6 months of hire

Driving may be necessary; proof of driver license and auto insurance required upon request

Applicable work experience may substitute for some education requirements
