New York RN Case Manager

A company is looking for an RN Case Manager.

Key Responsibilities

Coordinate all aspects of a patient's care, including medical, behavioral, functional, and social needs

Conduct ongoing care coordination and needs assessments for complex patients, developing individualized care plans

Participate in interdisciplinary team meetings and quality improvement processes while mentoring new hires

Required Qualifications

Current RN License in good standing in the state of practice

Bachelor's degree in nursing or an associate's degree in nursing with a bachelor's degree in another field

Minimum of 4 years of clinical experience, including 2 years in case management

Certified Case Manager (CCM) certification or commitment to obtain it when eligible

Geriatric care experience is highly desired
