Strong interpersonal & written communication skills. Ability to multi-task, work independently, & work efficiently in a fast-paced environment with changing priorities. Common types of patients & job duties : Dealing with all payers, uninsured, homeless. Completing initial assessments, DC planning (including nursing homes, home health, DME, prior authorizations, etc) Case load / patients per day / consults per day Caseloads can range from 15-28 on average EMR : Cerner Is this position for UR, discharge planning, or both? Discharge planning What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.) : Inpatient Experience or 'Must have' skills : 3 years' nursing experience in an acute hospital setting- Prior care coordination experience in a CommonSpirit / Dignity / CHI hospital Call Requirement : None Scrub Color / Dress : Business casual & lab coat.
