Title : Advanced Practice Provider (Nurse Practitioner or Physician Assistant)

Reports To : Chief Medical Officer

Location : Redding & Round Mountain, CA

Schedule : Monday–Friday, 8 : 30 AM – 5 : 00 PM

The Advanced Practice Provider (APP) delivers comprehensive, patient-centered primary care to individuals across all age groups, emphasizing preventive health, chronic disease management, and patient education - particularly for underserved populations. The APP collaborates closely with the integrated healthcare team to ensure high-quality, coordinated care.

Key Responsibilities

Patient Care

Provide comprehensive primary care across the lifespan using a team-based, holistic approach.

Perform patient assessments, diagnosis, treatment planning, and medication prescribing within the scope of practice.

Document care accurately and promptly in the Electronic Health Record (EHR).

Manage follow-ups for labs, imaging, and referrals.

Preventive Health

Promote wellness through education, screenings, and counseling.

Conduct health assessments, well-child visits, immunizations, and gender-specific care.

Collaboration

Consult with supervising physicians as needed per standardized procedures.

Participate in team meetings, peer reviews, and quality improvement initiatives.

Perform other related duties as required within the scope of practice.

Qualifications

Education & Experience

Master’s degree in Nursing or Physician Assistant Studies from an accredited program.

National certification (NP or PA) and active, unrestricted California license.

Minimum 1 year of clinical experience preferred; rural health experience a plus.

Leadership skills desirable.

Certifications

Current BLS and ACLS

Valid driver’s license

DEA license

Skills

Strong clinical, diagnostic, and treatment abilities.

Excellent communication and interpersonal skills for working with diverse populations.

Proficiency in EHR systems and Microsoft Office applications.

Sound clinical reasoning and knowledge of standard medical equipment.
