Community Wellness Advocate II - Hybrid

Job Title: Community Wellness Advocate II (Remote/On-Site)

Job Type: Temp to Hire

Schedule: Full time (40 hrs per week), standard 8am-5pm PST

Hourly Rate: $20-$25/hr depending on experience

Duties: Role will require heavy patient engagement involving reaching out via phone or messaging methods. Duties can include coordinating care for patients, sending messages, requesting referral information, reviewing records, etc.

**Role will be primarily remote but the candidate needs to be local as there may be some onsite needs.**

Education:-

Bachelor’s degree (Healthcare area of study is preferred) OR 3-5 years in a healthcare system environment.

Preferred Certifications & Licensure:
-Certification as a Medical Assistant by an accredited program.
-Licensed Vocational Nurse by an accredited program.
-BLS certification.
