Maine Licensed Population Health Coordinator

A company is looking for a Population Health Coordinator - Medical.

Key Responsibilities

Manage data and registry for special populations, assessing progress and facilitating care team collaboration

Coordinate care for complex members, providing direct support and training staff on collaborative care practices

Support individual member needs through outreach, education, and crisis management while documenting treatment plans

Required Qualifications

Registered Nurse (RN) or Licensed Practical Nurse (LPN) with SAMHSA required training for an X DEA license

Bachelor's degree in Nursing preferred

At least 5 years of experience in direct patient care in addiction medicine or related behavioral health fields

Understanding of care coordination, case management, and health care regulations

Proficiency in EMRs, Microsoft Office / Google Suite, and data analysis
