Proactive Titration LPN

A company is looking for a Proactive Titration Licensed Practical Nurse for weekend hours.

Key Responsibilities

Contact patients to discuss medication titration recommendations and ensure adherence to treatment plans

Maintain detailed documentation of patient interactions in compliance with HIPAA and clinical protocols

Provide timely and empathetic communication with patients and healthcare providers

Required Qualifications

Licensed Practical / Vocational Nurse (LPN / LVN) with compact licensure

5+ years of clinical experience in acute or ambulatory care, preferably in cardiology or nephrology

Experience maintaining HIPAA-compliant environments

Must have a HIPAA-compliant home office setup with reliable internet

Strong track record of managing time effectively and working independently
