Nurse Practitioner Opportunity – Primary And Palliative / Home-Based Care Locations : Fargo & Moorehead, ND About The Organization We're an independent, nonprofit, community-owned healthcare organization dedicated to helping people live fully, with comfort, meaning, respect, and dignity. Serving over 50 counties across North Dakota and Minnesota, they have over 40 years of history providing compassionate, patient-centered care. We offer a unique home-based care model, including primary care and palliative services, supported by full-time medical physicians and nurse practitioners who are on-call 24 / 7. The Role Seeking experienced Nurse Practitioners to support growing patient panels in Fargo. These are full-time positions with a mix of home visits and assisted living facility visits. Providers have autonomy in scheduling their patient visits and maintaining a balanced work-life rhythm. Responsibilities Provide high-quality primary and palliative care to geriatric patients in their homes and in assisted living facilities. Maintain autonomy over scheduling to best serve patient needs. Collaborate with medical team and participate in weekly team meetings. Patient load : 4–6 per day on average Qualifications Current Nurse Practitioner license in North Dakota and Minnesota. Minimum of 3 years NP experience; primary care with geriatric population strongly preferred. Palliative or home-based care experience highly valued. DEA registration required. Ability to work independently in patient homes and manage a flexible patient load. What They Offer Competitive salary : $105K–$135K / year (based on experience).
