Description :

This position involves travel as a Social Worker focusing on case management.

The unit typically has 20 beds and a team of Case Managers and Social Workers to manage the load.

Candidates must be adaptable to different units and possess strong communication skills.

Typical patient ratios are 1 Social Worker to 40 patients.

Candidates will conduct suicide risk assessments and substance assessments.

Discharge planning assessments will be a significant part of the role.

Experience in Emergency Department settings is preferred but not required.

Knowledge of palliative care and goals of care conversations is beneficial.

Ideal candidates are professional, dedicated, and friendly.
