Compact Licensed Case Manager RN

A company is looking for a Case Management - Registered Nurse.

Key Responsibilities

Assess, plan, implement, and coordinate case management activities to evaluate members' medical needs

Utilize clinical tools and apply clinical judgment to address complex health and social indicators

Collaborate with stakeholders to overcome barriers and enhance members' overall wellness

Required Qualifications

Registered Nurse with an unrestricted nursing license and multi-state / compact privileges

3+ years of clinical / nursing practice experience

1+ year of medical case management, care coordination, or discharge planning experience

High-speed internet access for remote work

3+ years of experience with personal computers and MS Office Suite applications
