Mississippi Licensed Care Manager

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and recommend care plans to address barriers to accessing appropriate care

Coordinate care between members, families, and providers to ensure person-centered services

Monitor member progress and update care plans as necessary to meet goals and needs

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in care management

RN - Registered Nurse State Licensure and / or Compact State Licensure required

Must reside in or around Mississippi for remote position
