Relocation reimbursement is available to those who are eligible to help move to Central Oregon for this position!

JOB SUMMARY :

The Mental Health Clinician will be a part of the Intensive In-Home Behavioral Health team to provides individual and group services as a part of a multi-disciplinary team of behavioral health professionals to provide 4-6 hours of intensive in-home services to children and families who need more services and supports than can be received in traditional outpatient settings. This multi-disciplinary team consists of a licensed medical provider, a clinician, a skills trainer, and peer support who work closely to reduce out-of-home placements for children at risk of hospitalization or residential treatment.

ESSENTIAL FUNCTIONS :

  • Works closely as part of a multi-disciplinary treatment team providing services in the home (which could include the home of their nuclear family, foster home, group home, etc.) to achieve the best outcomes for the clients;
  • Conducts comprehensive biopsychosocial assessments that includes assessment for the presence and impact of trauma (e.g., personal, intergenerational, community, and historical) in the youth and caregivers, and be able to provide and justify mental health diagnoses given;
  • Identifies needs and current functioning by working with youth and caregivers to comprehensively assess current strengths, behavioral health needs, and functioning across key life domains (e.g., school, vocation, family, social, and community);
  • Works with the youth and caregivers to prioritize the most critical behavioral health needs and concerns that will be the focus of treatment planning and delivery. If care planning includes other team members, such as in team-based wraparound or other care coordination models, prioritization will be conducted in collaboration with other team members as well;
  • Works with the youth and caregivers to conduct a functional analysis of antecedents and consequences of the youth's behavior to yield a functional understanding of behavior;
  • Works with the youth and caregivers to develop a treatment plan, written in the language of the family, with a manageable number (e.g. 1-4) of priority needs and goals. This should include measurable indicators of progress;
  • Works with the youth and caregivers to identify and address risk and safety concerns at home, in school, and in the community;
  • Develops individualized safety plans that should be co-created by the youth and caregivers, and include the identification of safety concerns, potential crises, triggers, de-escalation and coping strategies utilizing functional strengths, actionable stabilization steps, prevention measures, and youth- and caregiver-identified supports. Regularly monitor and update the safety plans;
  • Responds to crises as part of an afterhours crisis rotation in conjunction with other members of the team and / or the mobile crisis team;
  • Uses crisis de-escalation skills and demonstrate ability to effectively prevent or stabilize crisis situations. Work with the youth and caregivers to develop their own crisis de-escalation skills;
  • Utilizes evidenced-based interventions in a variety of therapeutic contexts including individual, family, and group therapy sessions;
  • Builds youth skills by working with youth and caregivers to develop adaptive and emotional coping skills such as emotional regulation, problem-solving, communication, conflict management, and decision-making;
  • Builds caregiver skills by working with caregivers to help them acquire and use behavior management skills as indicated by the treatment plan;
  • Engages the youth and caregivers in initial and continued psychoeducation surrounding the youth’s diagnoses and / or behavioral health needs, as well as applicable intervention strategies;
  • Ensures that there is cross-system collaboration and service coordination, including regular cross-system collaboration meetings;
  • Maintains documentation in client files and agency logs in EHR within agency-stated timelines;
  • Models appropriate professional behavior to staff, clients, and community partners;
  • Depending on IIBHT caseload size, it is possible this position would provide therapeutic services to children and families not involved in the IIBHT program;
  • Develops and maintains strong collaborative relationships with a wide range of community partners, including, but not limited to : County DHS and Health Departments, Adult Community Justice, Public Health, and other programs and departments;
  • Complete clinical chart reviews, as assigned;
  • Works closely with referral sources, keeping them informed on the progress of the clients, as needed and appropriate to client confidentiality per HIPAA and 42 CFR and Oregon statutes;
  • Other related duties as assigned by the Clinical Supervisor or Program Director.

  • Ensures that any required certifications and / or licenses are kept current and renewed timely;
  • Works independently as well as participates as a positive, collaborative team member;
  • Performs other organizational duties as needed.
  • Requirements : QUALIFICATIONS :

    EDUCATION AND / OR EXPERIENCE REQUIREMENTS :

  • Master’s degree in psychology, social work, counseling, behavioral sciences, or related field required
  • For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hire
  • LICENSES AND CERTIFICATIONS :

  • QMHP, CSWA, or PCA for MH Clinician I required (CS-8 pay grade) or
  • QMHP-R (Trainee on CS-7B pay grade) upon hire with ability / commitment to attain certification within 2 years
  • Must maintain a valid Oregon Driver License or ability to obtain one upon hire and be insurable under the organization’s auto liability coverage policy (minimum 21 years of age and at least 3 years without any DUI or moving violations)
  • PREFERRED :

  • Bilingual in English / Spanish a plus
