Christiana Care Health System is currently hiring a full time Physical Therapist to work at the Health Care Center Rehabilitation Services site located in Newark, Delaware. Site hours are Mondays, Wednesdays, and Fridays from 7 : 00 a.m. to 4 : 00 p.m and Tuesdays and Thursdays from 7 : 00 a.m. to 7 : 00 p.m.

Primary function :

To provide Physical Therapy for individuals requiring rehabilitation after injury or disease process.

Principal Duties and Responsibilities :

• Evaluate and re-evaluate patients in a timely manner.
• Establish appropriate treatment programs that include goals to be attained in a timely manner.
• Complete all documentation relating to patient care, including evaluations, progress notes, and discharge summaries.
• Review active patient records in a timely manner.
• Provide patient and family education and training.
• Participate in departmental organizational planning, in-services, educational programs, and community based marketing opportunities.
• Participate in mandatory meetings and hospital programs, seminars, and workshops.
• Maintain written and verbal communication with referral sources concerning patient care.
