JourneyCare Hospice is seeking a Nurse Practitioner to join our team! New competitive salaries, and immediate opening available!

Qualifications :

  • Current, unencumbered Nurse Practitioner (NP) license in the state of Illinois.
  • Minimum of 2 years of clinical experience; hospice or palliative care experience a plus.
  • Strong assessment, communication, and documentation skills.
  • Ability to work independently, manage time effectively, and adapt to a PRN schedule.
  • Valid driver’s license, auto insurance, and reliable transportation (travel required to patient homes and facilities).

HOURS : PRN As Needed Monday-Sunday 8 : 30AM-5PM for Face to Faces for Hospice Patients

TERRITORY : Crystal Lake, Johnsburg, McHenry, IL.

    • What You’ll Do :

  • Conduct Medicare-required Face-to-Face (F2F) encounters to determine ongoing hospice eligibility.
  • Perform patient assessments focused on clinical status, disease progression, and functional decline.
  • Document findings in accordance with CMS regulations and agency policies.
  • Collaborate with interdisciplinary team members, including physicians, nurses, social workers, and chaplains, to support care planning.
  • Communicate assessment outcomes clearly and effectively with both patients / families and care teams.
  • Provide clinical insight and education regarding disease trajectory and hospice appropriateness.
  • Maintain timely and accurate completion of all required documentation in the electronic health record.
