Care Coordinator

A company is looking for a Care Coordinator to facilitate client care and manage referrals in a behavioral healthcare setting.

Key Responsibilities

Serve as the primary contact for concierge care and manage referrals for Intensive Treatment Services

Track and document the lifecycle of all referrals to ensure timely outreach and placement

Develop relationships with external referral sources and coordinate with internal teams to enhance client care

Required Qualifications

High school diploma or equivalent required; associate's degree in a healthcare-related field preferred

2-3 years of experience in care coordination or referral management within behavioral healthcare

Familiarity with higher levels of care (IOP, PHP, inpatient) and behavioral health systems

Experience with electronic health records and case tracking systems

Remote work experience and familiarity with virtual care models is a plus
