About The Position

Job Title : Pediatric RN - Home Care

Profession : Registered Nurse

Specialty : Case Manager RN

Duration : 13 months

Shift : Monday - Friday

Hours per Shift : 8 : 00 AM - 4 : 30 PM EST

Experience : One year of experience as a clinical pediatric nurse strongly preferred, home care experience preferred.

License : Client Licensed RN

Certifications : BLS

Must-Have : Strong interviewing, assessment, and communication skills

Description :

Develop care plans appropriate to the patients’ and families’ health care needs in conjunction with patients, families, physicians, clinical operations teams, and third-party payers.

Complete the OASIS Assessment as needed to accurately reflect the clinical, functional, and social needs of the patient.

Ensure completion of all appropriate clinical records for compliance with state and federal legislation, facilitating seamless service within established time frames.

Refer to appropriate healthcare systems and community agencies / resources.

Communicate assessment / reassessment outcomes to healthcare team members to ensure appropriate follow-up based on the patient and family’s plan of care.

Display an exemplary level of patience, courtesy, and flexibility.

Interact with patients and staff to maintain positive relationships and to meet agency goals and objectives.

Ensure problem resolution at the point of contact.

Maintain appropriate productivity requirements.

Adhere to professional home visit performance standards.

Strong interviewing, clinical assessment, and communication skills are required.

Ability to exercise initiative and independent judgment while managing various assignments and completing assigned duties.

Possess a valid driver’s license and current auto insurance.

Travel throughout the service area year-round.

EPIC or Netsmart experience is highly desired.

Certified home health care experience is preferred.
