Job Summary:

As a Licensed Practical Nurse (LPN), you will play a vital role in our care team, working closely with patients, families, and other healthcare professionals to provide high-quality care. We are seeking a dedicated and compassionate LPN to join our team on a part-time and full-time basis.

Responsibilities:

• Administer medications and treatments as ordered by physicians

• Monitor and record patients' vital signs and medical conditions

• Develop and implement individualized care plans with input from the healthcare team

• Perform routine nursing tasks, such as wound care and vital sign checks

• Communicate effectively with patients, families, and healthcare team members

• Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements:

• Current LPN licensure in the State of New Jersey

• Associate's degree in Nursing or related field

• Minimum 1 year of experience as an LPN in LTC

• Ability to lift and transfer patients with assistance as needed

• Strong communication and interpersonal skills
