Multi-State Licensed Cardiometabolic Nurse Practitioner

A company is looking for a Bilingual Cardiometabolic Nurse Practitioner.

Key Responsibilities

Conduct assessments and virtual visits for patients with cardiometabolic conditions

Formulate diagnoses and develop individualized treatment plans, including medication management

Manage prescription refill requests and collaborate with the care team to ensure continuity of care

Required Qualifications, Training, and Education

Master's or Doctoral degree in Nursing; specialization in primary care, internal medicine, or geriatric medicine preferred

Active RN and APRN license in the state of practice, with multi-state licensure required

3 or more years of outpatient clinical practice experience, particularly in managing hypertension and hyperlipidemia

Proficiency in telehealth technology and electronic health records (EHR)

Fluency in English and Spanish preferred
